At Preferred Direct Family Care, we believe in full-price transparency. There are no hidden costs or fees, so you know what to expect.

Prices:

One-time Initial Enrollment fee: $89

Monthly Membership:

  • Children and Teens (3-19 years): $29 per month (with parent/guardian membership)

  • Adults (20-64 years): $59 per month

  • Seniors (65+ years): $89 per month

Family Plans:

  • $149 per month (maximum): Covers up to two adults and two or more children (legal dependents aged 19 or under) from the same household.

Group Pricing:

  • Available for Small Business Employers; please get in touch with us for more information.

Re-Join Fee: $150

Comprehensive Membership Services:

  • Physical Exams: Annual, school, sports, DOT.

  • Chronic Care Management: Manage stable conditions such as diabetes, hypertension, high cholesterol, hypothyroidism, depression, and anxiety.

  • Diagnostic Services: Electrocardiogram (EKG), urinalysis, rapid tests for strep throat and COVID-19, and basic vision/hearing screenings.

  • Procedural Care: Removal of benign skin lesions/warts, cerumen (ear wax), non-complex wound care, sutures (excluding facial), and abscess incision and drainage (excluding facial infections).

  • Laboratory Services: Drawing basic labs with access to discounted rates.

  • Radiology and Imaging: Access to these services at reduced rates.

Note: Additional fees apply for specific tests.

Without Membership:

  • Office Visit Fee: $89 per visit (In-Office, Telehealth)

  • Home Visit Fee: $250 (with/without membership)

  • Food Sensitivity Testing

    Food Sensitivity Testing

    The inability to break down certain types of foods can lead to anxiety, fatigue, gas, diarrhea, and other unpleasant symptoms. We test for 286 food antegens.

  • Environmental & Food Allergy Testing

    Symptoms can range from sneezing, itching, congestion, skin rash, gastrointestinal distress and, in extreme cases, anaphylaxis. Our IgE antibodies test evaluates 295 allergens from a vast array of environmental and food sources.

  • Heavy Metal Testing

    Symptoms can vary drastically for example abdominal pain, anemia, chills, diarrhea, muscle weakness, nausea or vomiting, shortness of breath, nervous system symptoms like tingling in your hands or feet, ect. We screen for the detection of 21 Toxic and 16 nutritional metals.
