• Same-day or next-day appointments via the Office, Virtual, or Telehealth

Monthly Fees: Individual (per member)

Ages:

3-19 years of age $9 per month (with patent/guardian membership)

20-64 years of age $59 per month,

>65 year of age $89 per month

- OR – Family Plans (per family) $129 per month max, (Family is defined as members of a shared household.) May include 2 adults and 2+ children (legal dependents age 19 or less).
